Kalıcı pacemaker ve internal defibrilatörler günümüzde
yaygın olarak uygulanmaktadır. Enfeksiyon ve disfonksiyon
bu cihazların hastalardan çıkarılmasındaki en önemli nedenlerdir.
Günümüzde bu cihazlardaki enfeksiyon insidansı %3'e
kadar düşmüştür, ancak enfeksiyon gelişimi bu cihazların
halen en mortal komplikasyonudur. Transvenöz yöntemlerin
başarısız olması, endokardit, vejetasyon ve trombüs bulunması
ve bazı elektrodların çıkarılmasında halen cerrahi tedavi gerekmektedir.
Bu çalışmanın amacı kalıcı pacemaker elektrodlarının
cerrahi olarak çıkarılması konusundaki deneyimlerimizi
sunmaktır.
Kliniğimizde Ocak 1990 - Aralık 1999 tarihleri arasında
yaşları 29-75 arasında değişen 6 hastaya cerrahi olarak kalıcı
pacemaker ekstraksiyonu uygulanmıştır. 5 hastada enfeksiyon,
1 hastada ise disfonksiyon nedeniyle ekstraksiyon uygulanmıştır.
Postoperatif dönemde bir hasta eksitus olmuş, diğer
hastalar halen asemptomatik olarak takip edilmektedir.
Enfekte kalıcı defibrilatör ve pacemaker elektrodlarının
çeşitli traksiyon yöntemleriyle çıkarılamadığı durumlarda cerrahi
tedavinin güvenle uygulanabiliceği g.rüşündeyiz.
Permenant pacemakers and internal defibrilators have a
wide spread use today. Infection and disfunction are the most
important reasons for the removal of these devices. Today the
incidence of infection is decreased to 3%, but infection is the
most lethal complication. Surgical removal is uncommon today,
but unsuccesfull transvenous traction methods, endocarditis,
the presence of vegetation and trombus and some type of
electrodes require surgical removal. The aim of this study was
to present our surgical experiments about the removal of permenant
pacemaker electrodes.
During the period between January 1990 and December
1999, 6 patients underwent surgical extraction of permenant
pacing electrodes. Patients’ age were within the range of 29-75
year. The reason of extraction was infection for 5 patients, and
disfunction for one patient. One patient died postoperatively,
the others are asymptomatic and are still in good condition.
We believe that surgical removal of these devices is a safe
procedure even for elderly patients who have complications
like infection, because of permenant pacemakers.
