Amaç: Koroner arter cerrahisi için başvuran hastalarda ilave
lezyon olarak karotid arterlerinde de lezyon olanlara
kombine yaklaşım ya da ayrı seanslarda yapılan cerrahi
girişimler ve sonuçla-rının karşılaştırılması.
Gereç ve Yöntem: Ocak 1995-Nisan 2000 tarihleri arasında
2086 adet koroner arter baypas cerrahisi yapıldı. Bu
hastaların preoperatif sorgulanması sırasında baş dönmesi,
kulak çınlaması, bulantı, senkop gibi karotid oklüzyonu
semptomları olanlara karotid doppler ultrasonografi
ve/veya karotid arter anjiografi yaptırarak koroner
arter hastalığı ile kombine karotid lezyonu bulunan 191
hastanın 118 tanesine (%61,7) aynı seansta koroner arter
cerrahisi ve karotid endarterektomi yaptık. 73 vakada
(%38,2) ayrı seanslarda koroner arter baypas ve karotid
endarterektomi yaptık.
Bulgular: Aynı seansta karotid lezyonuna ve koroner arterlerine
müdahale edilen 118 hastadan 6 tanesinde (%5) stroke
gözlendi, 3 hasta (%2,5) postoperatif erken dönemde
eksitus oldu. İki ayrı operasyonla koroner arter ve
karotide müdahale edilen 73 hastadan 4 tanesinde (%5,4)
stroke gözlendi ve 2 vaka ise eksitus oldu (%2,73).
Sonuç:Ateroskleroz sistemik bir hastalık olduğundan, koroner
arter cerrahisi öncesinde karotid sistemi sorgulanarak
cerrahi sınırlarda karotid lezyonu olanlarda koroner baypasla
aynı seansta veya ayrı seanslarda karotid endarterktomi
yapılmalıdır. Koroner arter hastalığı ile kombine
olarak karotid lezyonu olan hastalara kombine yada ayrı
seanslarda yapılan koroner arter cerrahisi ve karotid endarterektominin
morbidite ve mortalite y.nünden birbirine
bir üstünlüğü yoktur.
Aim: Our aim was to review the surgical options and their results
in the patients admitting for coronary artery surgery
accompanying carotid artery lesions.
Material and Method: Between 1995 January- 2000 April
2086 coronary aretry bypass grafting were operated in
our clinic. In the questionnairing of these patients dizziness,
tinnitus, nausea, syncope story positive ones were
selected for carotid doppler ultrsonography and/or carotid
artery angiography and 181 0f the 191 patients (61.7%)
who had carotid lesions accompanying coronary artery
disease underwent coronary artery surgery and carotid
endarterectomy in the same session. For the remaining 73
patients (38.2%) we performed these two operations in
two seperate sessions.
Results: Stroke was noted in 6 (5%) patients, 3 patients (2,5%)
died in the early postoperative period who were operated
for their coronary and carotid lesions in the same session.
Stroke (4 patients) and mortality (2 patients) rates were
5.4% and 2,73% respectively for the patients operated in
seperate sessions for their carotid and coronary artery diseases.
Conclusions: Since atherosclerosis is a systemic disease, before
the coronary artery surgery carotid system must be
evaluated and carotid endaretrectomy should be performed
in the same or a seperate session accompanying
coronary artery surgery for the ones who have carotid lesions
in the surgical indication borders. We concluded
that each of these options has no superiority to the other
one comparing the morbidity and mortality.
