Adnexal torsion is an infrequent cause of acute pelvic pain and is the result of the rotation of ovary, fallopian tube or both around their own ligamentous structures and accounts for 3% of all gynecologic emergencies. Although it can be seen at any age, it is most frequent in reproductive ages. Our case was a twenty four years old virgin and she presented to our clinic with complaints of severe pelvic pain. Transabdominal ultrasound showed an enlarged right ovary and a cystic mass which was 5 cm in size. Doppler ultrasonography showed some flow in the right ovary. Due to persistent pain and a presumed diagnosis of ovarian torsion the patient underwent Iaparoscopy; laparoscopic ovariopexy was performed. Laparoscopic ovariopexy should be considered in selected cases to prevent recurrence in ovarian torsion cases. Copyright © 2012 by Türkiye Klinikleri.
