Özet

Embolizasyon, kateter aracılığı ile değişik embolizan maddeler kullanılarak, damar veya damarsal

lezyonun kanlanmasının durdurulması için uygulanan girişimsel radyolojik tedavi yöntemidir.

Tümöral dokularda embolizasyonun amacı ise tümörün kan akımını durdurarak dokunun iskemisi ve tümörün

küçültülmesidir. Tümör emboloterapi; acil tümöral kanamalarda kanamanın durdurulması, hipervasküler

tümörlerde cerrahi öncesinde tümör kanlanmasının azaltılarak operatif kanama riskini

azaltmak, semptomatik tümörlerde tümörün kanlanmasının azaltılarak tümör boyutunu küçültmek ve

inopere tümörlerde palyatif amaçla uygulanabilmektedir. Amaç tümörün kılcal vasküler yatağının embolizasyonu

olduğundan tümörün besleyici damarlarında mümkün olduğu kadar süperselektif kateterizasyon

sağlanarak distal embolizasyon esastır. Embolizayon için kullanılabilecek çok çeşitli ajanlar

olmakla birlikte tümör emboloterapide genellikle partiküler embolizan ajanlar tercih edilmektedir. Transarteriyel

Kemoembolizasyon (TAKE) ise vasküler embolizan etki elde etmek, tümöral sitotoksik etki

elde etmek, vücudu kemoterapötik ajanların sistemik yan etkilerinden korumak ve daha az kemoterapötik

ajanla lokal maksimum etki elde edebilmek amacıyla geliştirlen özellikli bir embolizasyon işlemidir.

TAKE embolizan partiküller ile kemoterapötik ilaçların birlikte uygulandığı sinerjistik bir tedavi yöntemidir.

Embolization is an interventional radiological treatment method applied to stop the blood

supply of a vascular lesion by using different embolizing agents through the catheter. The purpose of embolization

in tumoral tissues is to stop the blood flow of the tumor for ischemia of the tissue and shrink

the tumor. Tumor embolotherapy can be applied to stop bleeding in acute tumoral hemorrhages, to reduce

the risk of operative bleeding by reducing tumor blood supply before surgery in hypervascular tumors,

to reduce tumor size by reducing blood supply in symptomatic tumors, and for palliative purposes

in inoperable tumors. Since the aim is embolization of the capillary vascular bed of the tumor, distal

embolization is essential by providing as much superselective catheterization as possible in the feeding

vessels of the tumor. Although there are a wide variety of agents that can be used for embolization, particulate

embolizing agents are generally preferred in tumor embolotherapy. Transarterial Chemoembolization

(TACE), on the other hand, is a specific embolization procedure developed to achieve vascular

embolizing effect, to achieve tumoral cytotoxic effect, to protect the body from the systemic side effects

of chemotherapeutic agents, and to achieve local maximum effect with less chemotherapeutic agents.

TACE is a synergistic treatment method in which embolizing particles and chemotherapeutic drugs are

applied together.
