Although it is rarely seen, unexpected perioperative deaths constitute an important medical and legal problem for anesthetists. It should not be forgotten that mortal perioperative myocardial infarction may be encountered, even in patients without prior diagnosis of coronary artery disease, or in patients with no prominent cardiac risk factors, so preventive measures should be employed. This report presents a case of intraoperative mortal myocardial infarction during a supracricoid partial laryngectomy operation, in a patient with no prior diagnosis of cardiac disease, except hypercholesterolemia and hypertension under control.
