Nursing care provided to elderly individuals through home visits is an important opportunity to protect and improve their health. This study aimed to exemplify the use of the Functional Health Patterns (FHP) model in nursing care provided to elderly individuals living alone through home visits. The case was monitored for six weeks with home visits organized by the nurse. The case was evaluated under the 11 patterns of the FHP model, nursing diagnoses were made and a nursing care plan was developed accordingly. The created care plan was implemented and evaluated through home visits. The use of the FHP model enabled a comprehensive evaluation of the elderly individual.
