Self- inflating bag-valve devices are commonly used for the ventilation of intubated patients, especially during resusitation and transport. These devices are generally safe, but minor deviations in their recommended use can expose patients to high airway pressures. Specifically, a tense oxygen reservoir caused by high flow rates of supplemental oxygen, decreased or occluded flow through the oxygen reservoir's bleed, or a combination of these can lock the bag in inspiration, prevent expiration, and thereby generate high airway pressures. In this case report, we present a patient in whom a sudden pneumothorax developed during ventilation with a bagvalve device. © 2012 Düzce Medical Journal.
